Ofloxacin

Tablet

Ofloxacin is a broad-spectrum fluoroquinolone antibiotic effective against Gram-positive and Gram-negative bacteria. It works by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes essential for DNA replication, transcription, and repair. Ofloxacin is widely prescribed for respiratory, urinary, gastrointestinal, and skin infections.

Mechanism

Ofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes required for DNA replication and repair. This disruption leads to bacterial cell death, making it bactericidal in nature.

Pharmacokinetics

  • Absorption: Rapid oral absorption
  • Distribution: Widely distributed in body tissues
  • Metabolism: Minimal hepatic metabolism
  • Elimination: Primarily renal excretion
  • Half-life: 5–7 hours

Side Effects

  • Nausea and vomiting
  • Diarrhea
  • Headache
  • Dizziness
  • Rare tendonitis or tendon rupture

Contraindication

  • Hypersensitivity to fluoroquinolones
  • History of tendon disorders related to quinolone use
  • Pregnancy and lactation (unless benefits outweigh risks)

Interaction

  • Antacids containing magnesium/aluminum (reduce absorption)
  • Warfarin (increased anticoagulant effect)
  • NSAIDs (increase risk of CNS stimulation/seizures)

Warning and Precautions

  • Monitor renal function during therapy
  • Use cautiously in elderly patients
  • Discontinue if tendon pain or inflammation occurs
